Do I have out-of-network coverage?
What is the deductible?
Is there coinsurance or copay required?
Is there a maximum number of sessions per year that are allowed?
Do you cover CPT code 90837 for outpatient psychotherapy sessions without requiring a prior authorization?
If so, what is the reimbursement rate?If you do not cover CPT code 90837 without a prior authorization, do you cover CPT code 90834 (which is a 45 minute session) without a prior authorization?
If so, what is the reimbursement rate for 90834?

Session Rates
If you do not wish to use insurance benefits for therapy, the following fees apply:
Intake session: $200
Standard session: $190
